Nomination Papers for Ed Thompson’s State Senate Candidacy can begin circulating as of June 1, 2010. People who would like to help gather signatures can print out the following form on plain white paper:

*Only adults residing in the 31st Wisconsin State Senate District may sign the forms. (See map on “Instructions”)
*Each name must be accompanied by the street address, municipality, and date. PO Boxes are not acceptable.
*Email is optional. People providing email address will be added to the campaign email update list.
*Circulator must add their name, address, date, and signature in the “Certification of Circulator” section.
*DO NOT NUMBER PAGES. That section is for campaign use.
